Founded in 2015, Delaware Care Collaboration (DCC) is an ACO, in which Saint Francis Hospital and the Medical Society of Delaware have come together voluntarily to give coordinated high-quality care to their patients. The ACO’s goals are to advance the health of populations, improve the patient care experience, and reduce the cost of care.
DCC has participated in the Medicare Shared Savings Program (MSSP) since 2016 and was recently approved by Delaware's Department of Medicaid and Medical Assistance (DMMA) to launch its Medicaid ACO in 2021. In 2018, DCC achieved the 12th highest savings rate percentage nationally among 548 MSSP ACOs.
DCC and Saint Francis Hospital are members of Trinity Health Mid-Atlantic regionally and Trinity Health nationally.
